Hazel Evangeline Miller, age 13, passed away on October 24, 2026, at Riley Children's Hospital in Indianapolis, IN. She was born, raised and adored in the Southwest community of Fort Wayne, IN.
Hazel was a student at Summit Middle School and was active and baptized in her family's church, The Chapel.
In addition to her parents, Chris and Kasie Miller, Hazel is saving seats in Heaven for her two brothers, Holden Miller and Calvin Miller; maternal grandparents, John and LaNae Abnet; her paternal grandparents, Brad and Mary Jane Miller; and a countless number of cousins, aunts, uncles, and friends who are as beloved as family. She'll also be advocating for an updated pet policy on behalf of her precious pup, Lemon.
Hazel approached life with both tenderness and tenacity, never believing any goal too lofty to attain, or challenge too great to overcome. She was unapologetically independent, yet a deeply loyal daughter, sister and friend. She was a talented artist and performer who created, danced and sang beautifully. An avid reader and excellent student, she was brilliant with words and maintained her sass, wit and wisdom through even the most harrowing of moments. Every day for her was a stage, and she dazzled--with a fantastic sense of style--on each and every one. She left the world sweeter and more lovely for everyone who has the privilege of knowing her. And until we join her, we are certain she'll be fine-tuning the choir and redecorating the halls of Heaven with a touch of pink--whilst holding a sushi roll.
A celebration of life service will be held at and live-streamed from The Chapel at a later date. The family requests donations be made to chadtough.org in Hazel's honor in lieu of flowers or gifts. To sign the online guestbook, go to www.fairhavenfortwayne.com
